POSITION SUMMARY:
To plan, organize and direct diverse programs of treatment, counseling, screening and education for CCAC clients in a variety of settings; (in homes, schools, nursing homes).
PRIMARY RESPONSIBILITIES:
• Provides physiotherapy services to CCAC clients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of client’s condition.
• Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.
• Ensures that clinical records are maintained in accordance to guidelines in Physiotherapy Procedure Manual.
• Responsible for scheduling clients and visits according to Physiotherapy priorities.
• Functions as a member of a multi-disciplinary team and maintains liaison with other health care professionals.
• Acts as an education and resource person for other health care professionals.
• Participates in the orientation of C.C.A.C. staff.
• Participates in committees as approved.
• Partakes in professional development activities and maintains knowledge and ability in the realm of physiotherapy.
• Maintains supplies and recommends equipment for department use within budget limitations.
• Interprets the role and objectives of the C.C.A.C. Program in the community.
EDUCATION AND EXPERIENCE:
• Degree or diploma in Physiotherapy and registered with the College of Physiotherapy of Ontario;
• Minimum two (2) to three (3) years of recent clinical experience;
• Community experience working with children and the elderly is considered an asset;
• Valid CPR certificate.
KNOWLEDGE:
• Medical conditions and cognitive/psychiatric disorders
• Client centered, multi-disciplinary team approach to client care
SKILLS AND ABILITIES:
• Ability to maintain a positive attitude in stressful situations
• Ability to organize daily workload in the presence of frequent interruptions
• Ability to establish rapport and maintain empathetic and therapeutic relationships;
• Ability to function effectively as a member of a multi-disciplinary team;
• Ability to work independently while exercising sound professional judgment;
• Fluency in English and French is required.
• A valid Ontario Driver’s License and access to a motor vehicle are required.
