Position Summary |
This community-based position will focus on cardiac and pulmonary rehabilitation and address chronic disease management within the context of a interprofessional primary care team. The position will be responsible for the operation of a cardiac and pulmonary rehabilitation program in the community setting, which will involve the provision of direct clinical services and oversee the integration of these services into the primary care home. Through a partnership with UBC Department of Physical Therapy, the incumbent will provide clinical education and training, orientation, supervision, and evaluation of physiotherapy students involved in the delivery of the cardiac and pulmonary rehabilitation programs designed to meet the rehabilitation needs of the public.
With a focus on improving the functional independence, knowledge, and self-management capacity of individuals and their families, the incumbent will also support the integration of cardiac and pulmonary rehabilitation services with Northern Health’s primary care approach. This position will report to the Director of Community Services, Prince George and will operate under a matrix leadership structure involving the UBC Department of Physical Therapy and the YMCA of the North. The incumbent will be a part of, and seek direction from, an inter-agency advisory committee to the Prince George Cardiac and Pulmonary Rehabilitation Program. |
Qualifications |
– Degree or diploma in Physical Therapy from an approved school. Registered with the College of Physical Therapists of British Columbia and eligible for membership in the Canadian Physiotherapy Association. – Two (2) years experience in acute care, long-term care facility or a community setting. – Previous experience in medical equipment assessment, including mobility equipment. – Valid class 5 BC Drivers license and use of personal vehicle.
