Clinical Nurse Specialist – Diabetes
Position Summary:
|
- To provide effective leadership in the area of specialty, improving clinical outcomes for selected patient populations through advanced nursing practice, program planning and delivery, consultation, research and its application, education, efficient resource utilization and capacity building.

Responsibilities:
|
- Provides direct, expert, comprehensive care/nursing services, incorporating health promotion, disease prevention and management, patient and family advocacy, and collaborative strategies to positively affect health outcomes.
- Provides consultation to nursing staff, clinical managers, physicians, and other team members regarding clinical and professional practice issues.
- Coordinates outpatient diabetes care to facilitate continuity of care for the diabetes patient population, Type I, complex Type II and supports diabetes care in the Gestational Diabetes Clinic.
- Provides education and support for the patient who uses Intensive Diabetes Management (MDI, CSII) including Pump Therapy.
- Participates in and provides leadership on interprofessional committees related to development of policies, procedures, protocols, guidelines, clinical pathways, standards of care, medical directives, education and research as they pertain to the diabetes patient population.
- Enhances the scientific base of nursing practice by utilizing and supporting research in nursing and with other disciplines.

Qualifications:
|
- Masters Degree in Nursing (candidates with a Masters degree in progress will be considered)
- Bachelors of Science Nursing Degree
- Minimum of 5 years recent and relevant clinical nursing experience, including medicine, and/or chronic disease prevention and management experience
- Minimum three (3) years recent and relevant diabetes management experience in the context of other chronic conditions
- Registered with the College of Nurses of Ontario
- Certified as a diabetes educator
- Knowledge and experience with patient self-management support
- Proven ability in independent and collaborative decision making
- Strong communication skills – oral, written, electronic
- Ability to work with adult learners and colleagues in a variety of settings using a collaborative approach
- Demonstrated commitment to own continuing academic growth and development
- Excellent analytical and problem solving skills – ability to respond to shifting priorities and to multi-task
- Strong organizational skills with demonstrated ability to meet deadlines/timelines
- Demonstrated computer literacy
- PREFERRED:
- Member of Registered Nurses Association of Ontario (RNAO)
- Member of the Diabetes Educator Section of the Canadian Diabetes Association
- Canadian Nurses Association (CNA) and/or related certifications
- Experience with clinical documentation in an Electronic Health Record
- Recent and relevant diabetes management experience in the context of diabetes and pregnancy
