Rehabilitation Case Manager – Full Time – Dartmouth/Halifax, NS

The Rehabilitation Case Manager provides services to injured and/or ill clients within the Dartmouth/Halifax geographical area. The services will include, but are not limited to, the coordination & implementation of equipment/services for hospital discharges, ADL assessments, ergonomic assessments. Field Case Management refers to both medical and vocational case management aimed toward safely, expeditiously and cost-effectively returning an injured or ill individual to their pre-injury and/or maximum medical improvement level, and their pre-injury job.

Key responsibilities:

• Meet with clients in their homes, physician’s, therapist’s, or attorney’s offices, and/or work sites to provide on-going case management services.
• Communicate both in person and by telephone with clients, medical providers, attorneys, employers, insurance carriers, etc.
• Assessment and implementation of required services and equipment to ensure an efficient and safe hospital discharge to home. Coordination with hospital personnel, physician, client and client’s family following approval for recommendations, as required.
• ADL and ergonomic assessments; recommendations and implementation of same following approval by customer.
• Co-ordinate and/or conduct on site Job Analysis of client’s pre-injury position as well as other positions requiring analysis as part of casework.
• Prepare reports and other required paperwork as documentation of all casework activities at a consistently high standard and in a timely manner.
• Understand and apply all special instructions required by individual insurance carriers and referral sources.
• Testify if called to do so, to substantiate any casework or reports that may be seen as relevant on hearings involving client.

Qualifications:

• Academic degree in Occupational or Physical Therapy, Kinesiology, or Nursing with a current license for practicing in NS
• Valid driver’s license
• Excellent verbal and written communication skills
• Proficient at Microsoft Word Office products
• Proficient at typing reports/correspondence
• Ability to work from a home office with reliable internet set-up
• Current licensing/credentials for specific rehab profession required
